Overview: The position of Nurse Practitioner - PRN entails
performing thorough health evaluations for adults and seniors in
their residences or skilled nursing facilities. This role aims to
enhance the quality of care and pinpoint potential health risks.
Collaboration with physicians and care teams is essential, along
with administering tests and maintaining documentation in
accordance with clinical protocols. The role offers flexible
scheduling and competitive compensation based on the number of

positively to the community! Responsibilities: About the Role
During visits that may last up to one hour, our providers assess a
member’s current health status, medical history, medication
compliance, social circumstances, and other risk factors. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Comprehensive Health Assessment (CHA) enhances care quality and
allows us to address multiple care gaps in a single appointment.
Our providers are responsible for: Conducting assessments for
adults and seniors, including medical history, diagnosis,
treatment, health education, referrals to physicians, case
management, follow-ups, and thorough documentation in line with our
guidelines. Administering and ordering necessary point-of-care and
laboratory tests. Collaborating with physicians, case managers,
social workers, family members, key caregivers, and other medical
personnel as needed. Working alongside Primary Care Physicians
(PCP) to educate patients and follow up as required. Providing
services at the member’s residence, whether at home or in Skilled
Nursing Facilities. Benefits of Joining Our Team: Flexibility –
Schedule visits according to YOUR availability. Assessments are
conducted from Sunday to Saturday, between 8 am and 7 pm. You can
choose to work during the day, evening, or weekends based on your
preferences. Competitive Compensation – Your earnings are based on
the number of assessments you complete. Increase your income
potential by being available for more visits and traveling to see
members outside your home area. Those averaging 20 to 100 visits
monthly can earn between $1,800 and $11,500 , based on a
per-assessment rate of $90 to $115 , depending on your state.

thorough onboarding training, you will be assigned a preceptor,
receive personalized feedback, and have free access to continuing
education opportunities. Licensure Support – If needed, we will
assist you in obtaining additional state licensure and
credentialing in neighboring states or others you may wish to visit
while conducting health assessments. Full-Time Opportunities – Our
PRN providers have the option to transition to full-time roles
supporting their home location or a specific state or region, along
with eligibility for a full-time sign-on bonus. Qualifications:
Essential Skills & Experience A Master’s Degree or equivalent
experience, along with successful completion of NP or PA licensure.
Current NP or PA licensure in the state of practice, including
prescription authority or the ability to obtain it. Board
certification from NCCPA, AANP, ANCC, or AACCN in Adult, Adult
Gerontology Primary Care, Adult Gerontology Acute Care, Family,
Emergency, or Acute Care specialties. Current BLS, ACLS, or CPR
certification. Preferred one year of experience as a Nurse
Practitioner or Physician Assistant; new graduates are encouraged
to apply! A valid state driver’s license and proof of adequate
automobile insurance for your state of residence. Strong computer
skills and familiarity with health/medical record software.
