Job Description:
Join Ascension, a leader in faith-based healthcare, as an
RN-Patient Navigator, where you will have the opportunity to make a
substantial impact on patient lives through personalized care
coordination and innovative support. This role is pivotal in
guiding patients through complex healthcare systems, focusing on
creating seamless healthcare experiences tailored to individual
needs. By leveraging Ascensions extensive network of facilities and
medical professionals, you will act as a crucial link between
patients and their care, enabling better health outcomes and
enhanced patient satisfaction. Assess patient needs and develop
personalized care plans. Navigate patients through the healthcare
system, ensuring they receive timely medical care and support.
Facilitate communication between patients, family members, and
various healthcare professionals. Monitor patient progress and
adjust care plans as needed. Educate patients and their families on
illnesses, treatment plans, and healthcare resources. Advocate for
patients’ needs and preferences within the healthcare setting.
Maintain comprehensive documentation of patient interactions and
care provided. Strong interpersonal and communication skills.
Comprehensive knowledge of healthcare systems and policies. Ability
to multitask and manage time effectively. Empathetic and
patient-centered approach. Proficiency in electronic health record
(EHR) systems. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Valid Registered Nurse (RN) license.
Certification in Patient Navigation is preferred but not
required.
