Job Description:

states. We are seeking a compassionate and skilled Licensed
Practical Nurse (LPN) dedicated to providing comprehensive care in
alignment with our mission to promote wellness and holistic health
in the senior population. As a member of the THSC team, you will
work within a dynamic interdisciplinary team to ensure the highest
level of care, support, and life quality for our residents. Your
clinical expertise and dedication to elderly care will play a
critical role in embodying our commitment to excellence in serving
those in the latter stages of their lives. Provide direct nursing
care to residents in accordance with individualized care plans.
Monitor patient health and maintain accurate health records in
compliance with state regulations and organizational policies.
Administer medications and treatments as prescribed by the
healthcare provider. Assist in the implementation of patient care
plans, including assessments and evaluations. Collaborate with a
multidisciplinary team to facilitate integrated and comprehensive
care. Educate residents and their families on disease prevention
and health maintenance. Participate in the development and updates
of care plans and health assessments. Strong practical nursing
skills and a deep understanding of patient care Ability to
administer medications and perform necessary treatments Excellent
observational and communication abilities Strong documentation
skills and proficiency in medical record software Medical Insurance

license to practice as a Licensed Practical Nurse (LPN) CPR/BLS
